A collaboration between the College of Pharmacy and the College of Nursing was on display at the American Diabetes Association 82nd Scientific Sessions in New Orleans from June 3-7.
James Sterrett, associate professor of clinical pharmacy and outcome sciences (CPOS) at the MUSC College of Pharmacy, and Terri Fowler, associate professor of nursing practice and director of interprofessional programs at the MUSC College of Nursing, presented the poster A Community -Academic Partnership to Optimize Outcomes in High Risk Underserved Patients with Diabetes on behalf of their work to provide comprehensive diabetes management to patients at Fetter Health Care Network (FHCN). Erin Weeda, assistant professor of CPOS at the college of pharmacy, is also part of the project.
They were presenting the most recent results of a grant awarded in 2019 from The Duke Endowment to fund a 3-year program that provides comprehensive diabetes management to patients at three FHCN health centers located throughout the Lowcountry. The program, designed to integrate an interprofessional health care team in FHCN's primary care settings, addresses the comprehensive health care needs of patients diagnosed with diabetes. The team, including a primary care provider, nurse practitioner, pharmacist, dietician, case manager, and population health specialist, focuses patient care on lifestyle modifications, physical and mental health, pharmacological treatment, health promotion, with the goal of preventing and treating associated comorbidities.
