All students applying for disability services must complete the "Request for Accommodation Form". This form needs to be submitted to the Department of Diversity, Equity and Inclusion attention: Stephanie Price, Director of Equity, EEO and University Accessibility Services. Processing for your request may take up to 4 weeks.
MUSC’s Medical Verification Clinical Form should be completed by the student’s qualified healthcare provider when requesting accommodations in the clinical setting. The student’s healthcare provider will need to complete this form as well as provide supporting documentation which should include testing and/or evaluations to support the student’s diagnosis. Students looking to apply for accommodations will also be required to complete the Student Accommodation Request Form. Accommodations approved for the clinical setting do not automatically transfer to the classroom (didactic) setting, therefore a new request for accommodations should be submitted to the Director of Equity, EEO, and University Accessibility.
