To request a SmartStream ID and password you need to submit a security request form .  If you are a new user, please note that you must have an e-mail ID before you can be assigned security for SmartStream. 

You can fill out the request online but you must print it out for appropriate signatures. Have your request approved by your department head or chairman (not the business manager).  If you are the department head or chairman, have your form approved by the appropriate dean or superior officer/administrator. 

Return the original form to Systems & Procedures (Attn: Sally Bridges), 408 Harborview Tower, P.O. Box 250824 (please note that fax copies cannot be accepted).
