Who can refer patients?
- MUSC-affiliated attending physicians. Referrals from mid-level providers or resident physicians shall be signed by an attending physician.
What is needed prior to referring (scheduling) a patient?
- A signed Collaborative Drug Therapy Management (CDTM) agreement between the attending physician and pharmacist(s). NOTE: A CDTM agreement needs only to be established once; it is not needed for each individual patient referral.
- A completed referral form. Inpatient referrals shall occur 1 - 2 days prior to discharge to allow for timely follow-up scheduling.
