* Services are provided from a central location from 4:00 pm-11:00 pm by Clinical Pharmacists ** Clinical Pharmacist covers centrally from 7:00 am-7:30 am *** Services are provided from a central location from 3:30 pm-11:00 pm by Clinical Pharmacists     All pharmacy services centralize from the hours of 11pm-7:30am .  The nursing units should utilize the same phone and fax numbers at ALL times.  Pagers will be changed to “out of hospital unavailable” from 11pm-7:30am .  The Clinical Pharmacist staff will ensure our faxes, phones, and pagers are adjusted at 7:30 am and 11:00 pm .
