Credentialed pharmacists participate in collaborative drug therapy management
with our physicians and develop collaborative drug therapy management
protocols which are used in collaborative practice. Credentialed pharmacists make decisions
in dosage adjustments of the Direct Thrombin Inhibitors when designated by approved prescribers on the DTI
order form. Credentialed pharmacists may make adjustments to dosages without the cosignature
by an approved prescriber. |
