Practitioner Credentialling

Application Process:

Precisely following the below listed outline will help to quickly and successfully complete the process.

Getting started:

• Request to obtain practice privileges for MUSC from CON, Office of Practice          
  
• Pick up “Form Letter” from CON, Office of Practice addressed to Department chairman
  
• Download “Chairman’s Request Form” (see link above)
  
• Turn in the completed form to chairman’s office
  
• Return signed copy of “Chairman’s Request Form” to CON, Office of Practice

Forward the following documentation to CON, Office of Practice. MUSC Medical Staff Office will not accept any incomplete application.

Documents needed:

• Current photo
• Current CV
• Copies of current licensures
• Copies of diplomas
• Protocol
• Documentation of Faculty Appointment (when applicable)

Practitioner Reports

Quarterly Reports

We are in the process of incorporating this report within the CON employee database. Until this process is finalized, please continue to forward Quarterly Report to the Office of Practice as a hardcopy.

Annual Reports

Annual Reports and revised Business Plans are due June 1.
Please attach all applicable documentation. Reporting instructions and format can be downloaded from CON Intranet - Practice - FORMS
