There are four ways you may be able to decide on changes affecting
your benefits plans:
- As a new hire, not only can you select your
benefits but you can also change your mind within 31 days of the
date of hire.
- Change Health Insurance Carrier
- Add/Drop health or dental coverage for yourself or dependents
- Add/Drop Optional and Dependent Life, Supplemental Long
Term Disability, or Long Term Care Insurance
- During Open Enrollment: This period of time,
usually four weeks in October, allows you to do the following
changes that will be effective the on the first of the following
January:
- Change health insurance carriers
- Add/drop health or dental coverage for yourself or dependents
- Set up MoneyPlu$ Dependent Care and/or Medical Spending
Accounts for the following year
- Year round. The following changes are allowed
anytime during the year:
- Change HMO Physician
- Drop Dependent Life
- Drop Supplemental Long Term Disability
- Drop Long Term Care Insurance
- The following changes are allowed anytime during the year
with :
- Add/Increase Dependent Life Insurance (medical evidence
of good health required)
- Add/Increase Supplement Long Term Disability (medical evidence
of good health required)
- Add/Increase Long Term Care (medical evidence of good health
required)
- Change beneficiaries
- Otherwise, changes are only permissible when you experience
a life qualifying event. Changes need to be made
within 31 days from date of event. Such qualifying events
include:
- Marriage
- Separation/Divorce
- Birth or Adoption
- Death
- Employment Change of Spouse
- Dependency Change for Children
To submit your changes in benefits, you need to fill out the Notice
of Election (NOE) form. Print it out and bring it to your Benefits
Specialists, Room 101 Harborview Tower. |
