Completion of the First Year Curriculum

• Change to Advisory Committee - add or drop member (pdf)
• Program of Study (pdf)* [COMPLETE BEFORE SCHEDULING QUALIFYING EXAM]

After Written and Oral Qualifiers

• Admission to Candidacy (pdf)*
• Plan of Research (pdf)* - NOTE: Submit form with a copy of the title page

Defense Forms

Once an Advisory Committee is formed it must meet at least once a year
