• Plan of Research (pdf)* - NOTE: Submit form with a copy of the title page
• Thesis Defense Notification (pdf)*
• Successful Defense (pdf)*
• Thesis Defense Rubric (pdf)

Once an Advisory Committee is formed it must meet at least once a year.
