Training Grants

1. Mandatory Health Insurance - either waive out of Student Insurance Plan or set up Account at Insurance web site. Students will be automatically billed unless they have waived out by the deadline date. (See section on Health Insurance)
   
2. Two step Intradermal TB Skin Test within 3 months of arrival to the University. If a student tests positive for TB then a copy of a chest x-ray after the student tested positive. This must also be done yearly.
   
3. Positive Measles IgG, Positive Mumps IgG, Positive Rubella IgG titers or signed documentation of two MMR vaccines. The vaccines must include date/month/year and be signed by a health care provider. Parental signatures are not acceptable. If they have titers as proof of immunity the student must provide an actual copy of the lab results.
   
4. Tdap (tetanus/diphtheria/Pertussis) or Td vaccine within the last ten years. This must include date/month/year and be signed by a health care provider.
   
5. Positive Varicella IgG Titer or Two Varicella Vaccines. The vaccine must include date/month/year and be signed by a health care provider. If they have a titer as proof of immunity the student must provide an actual copy of the lab results.
   
6. Positive Hepatitis B Antibody Titer. The student must provide an actual copy of the lab results. The student can also start the vaccine here after classes begin.
