If you intend to issue a survey for your area please contact the Systems and Procedures department with the following information:
1. The area that is issuing the survey.
2. The expected start and end dates.
To avoid confusion among the customer base, we recommend that only one area be conducting a survey at any given time. If you have plans to issue a survey, you can review the survey calendar by month to determine which areas are conducting surveys in which time slots.
After you determine the dates you will be using, please e-mail your survey
