Please provide the user's name, SmartStream ID, phone, and room/building location.  In addition, we need to know if the user will be entering requisitions and/or blanket order releases online.  You must select at least one workgroup manager.  This is the individual who will manage the requisition entry workgroup online by adding/deleting users from the workgroup and ensuring that workgroup tasks are completed.  You may have more than one workgroup manager.
