Success in Lancaster County!
Diabetes has long been a major health problem in South Carolina.  Health care providers in Lancaster County decided to act.  Months of research and grant writing resulted in funding to create a Diabetes Education Center for county residents with a certified diabetes educator and a comprehensive curriculum.  Clerkship medical students rotating with Lancaster preceptors have used their community project requirement as a vehicle through which they have injected their energy and effort into all phases of the Diabetes Education Center initiative . . . from initial research, to curriculum development, to efforts for future funding.  Student projects as follows:
1999
December - Improve management & treatment of DM patients at Palmetto Tri-County Internal Medicine.  Conducted chart audits to determine level of DM care & compliance/  Created database of DM patients in practice - for future use in Diabetes Initiative.
2000
February - Continue updating DM patient database at Palmetto Tri-County IM.  All patients with DM code seen by providers from Jan 97 - Sep 99 were identified and letters were sent to 300 patients who were due for an office visit.
March - Creation of flow sheet for Palmetto Tri-County IM patients - beneficial to providers and comprehensible to patients. Patient handout created - guide to understanding flow sheet.

May - Development of Diabetes Education Center (grant writing, budget development, statistical research)Survey with cover letter mailed to 500 random county residents.
June - Collect and analyze returned surveys, continue statistical research & create statistical database.  Survey statistics collected as of June.

July - Improve DM screening & care of patients at Barnett Family PracticeSurvey staff and implement methods to improve ADA guideline compliance.
December - Ensure Palmetto Tri-County IM DM patients receive timely foot screens through chart audits, provider & staff input.
2001
May - Evaluate effectiveness of Diabetes Education Center's patient education - by tracking & analyzing HBA1C's for DM patients at Palmetto Tri-County IM.
June - Identify & inform providers of most frequent patient misunderstandings about their DM care.  Surveys: providers and patients.

Newberry County is serious about smoking cessation!

Health care providers in Newberry County have long been concerned about the effects of smoking in their county’s population.  Clerkship medical students rotate with preceptors in Newberry County and lend their efforts to the smoking cessation iniative.

Press Release:  The SC Tobacco Collaborative produced “Hot Off the Grill” to generate awareness about the dangers of secondhand smoke, an often- overlooked public health issue.

Clerkship Student Requirement
  • Implementation/Participation: Each student participates in a community or practice health improvement project that encompasses a population-based health care approach and uses Continuous Quality Improvement (CQI) concepts and tools to measure effectiveness.
  • Resources/Assistance: SC AHEC community site coordinators assist students by providing guidance and resources on community needs / existing programs.  Coordinators are very knowledgeable about CQI and provide ongoing assistance throughout the rotation.
