Forms 2008-2009
Below are forms that are part of the financial aid process. Secure online forms are available (for current students or for students with a NET ID (issued by MUSC). (form availability indicated by red checkmark)

Secure Online Forms are submitted online securely with SSL (Secure Sockets Layer) 128-bit encryption on a secure server located at the Medical University of South Carolina. Fillable forms can be filled out on the screen, then printed and faxed or mailed.

Students will be contacted by the Office of Financial Services if any of the other forms listed will be required for processing aid for the 2008-2009 academic year.

New Students without a Net ID number from MUSC can only complete the fillable version.

If you are unable to access a form in a PDF format please call 792-2536 in the Office of Financial Aid Services to have a form mailed to you.

* New Students applying for a Stafford / Graduate PLUS will need to contact the lender of your choice and complete a Master Promissory Note.  Please indicate our Federal School Code 003438.

* Renewal borrowers, students with an outstanding balance on a previous student loan, may choose to retain their current lender or select a new lender.  However, you may select a new lender if you have not been satisfied with the service you have received or prefer the options, services, and/or benefits of another lender.
